Physical Examinations:

Students registering for Athletics must have a current physical examination on file before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be handed into the Athletics Department for initial processing two weeks prior to the start of the season.

Physicals are valid for 12 months from the date of the last exam and must not expire prior to the end of the participating season.

RIIL Assumption of Risk Forms:

A New RIIL Assumption of Risk (AOR) form must be submitted to be able to participate in PHS sports.  One per four years is all that is required.  The new AOR form released SY20-21 is the current accepted form.  If your student participated in sports in the SY20-21 and have handed in the new AOR form they are all set for four years.  All paperwork must be submitted to the Athletics office before practice/tryouts begin.
